Thank you for registering for our upcoming Medication Aide Class at Triad Medical Academy. Please read the instructions carefully to complete the registration process.  

Transcripts

We will need a copy of your high school, college OR GED transcripts for your file.  It does not have to be official. Here are a few options to obtain your transcript:

  1.     You can call or visit your high school or college directly to order your transcripts.
  2.     If you went to high school in Guildford County you may order one at this website : https://guilfordnc.scriborder.com
  3.     For a High School transcript, you may order one from this site:  https://www.needmytranscript.com
  4.     If you are currently enrolled in, taken some college courses, or graduated from college you may order your transcripts at National Student Clearinghouse (if your school participates) Click here:   https://tsorder.studentclearinghouse.org/school/select

Other Documents

We will also need your Social Security Card, valid Driver License or Photo ID, for identity verification.

Dress Code and what to bring.

You may wear any color nursing uniform or scrubs to class and closed toe shoes.  Also bring a black pen, highlighter, and a notebook or notebook paper to class.

From Wendover Ave, turn on to Clifton Rd, take the first right on to Oak Branch Drive, (at Logan Systems Building) Then take the 1st Right in to the Parking Lot, and our building is the first building on the left. Building 2.

No Make-up Days

There are no make up days for this course. You must attend all days/hours to pass the class or re-register and pay tuition again.
